If the answer to any or all of these questions is yes, then 4C is happy to announce that we have the capability of electronically transferring the claim amounts into your checking or savings account. If you sign up for direct deposit, we will transfer the funds as soon as we have finished processing the funds from the state without having to take the time to run a check, stuff the check in an envelope, and drop the check in the mail.

To sign up for direct deposit of claim checks, please complete the information on the direct deposit form, have an authorized person at your bank or financial institution sign the bottom part, and return the form to 4C by mail or with your next month's menu.
